Personal Care
Personal Care, formerly termed Assisted Living, is ideal for people who can use just a bit of assistance with life's daily activities, but do not need the fulltime care offered with Skilled Nursing. Personal Care living emphasizes privacy, independence, individual choice and self-direction. Our overall goal is to provide a safe, secure and caring, home-like environment, while preserving and reinforcing individual autonomy, dignity and self-worth. |

Skilled Nursing
Skilled Nursing services are offered to persons requiring 24-hour care and supervision under the direction of a registered nurse. Each room is equipped with a bed, chest of drawers, bedside cabinet, and space for clothing and personal items. Residents are encouraged to bring other items as space permits to help make their environment more like home. |

Short-term Rehabilitation
Our comprehensive rehab program operates within an environment that focuses on all of a resident’s physical, social and spiritual needs. An individualized treatment plan is developed soon after admission to assist residents in meeting the personalized goals of their rehab program and to assure residents’ comfort and involvement in their care. In addition to a detailed therapy plan, fun and laughter are important components of treatment. Rehab staff and residents work together in a relaxed, friendly environment. Outpatient Rehabilitation
Our skilled rehabilitation team is available also to individuals who are living in their own homes, whether in our Fairmount community or the surrounding community. We offer Physical, Occupational, and Speech therapy at both Wheat Ridge and Crest View. Both areas host a fully-equipped therapy gym that enables residents to gain strength and regain skills. Memory Support
Heartwood, Fairmount's special care residence for persons with memory loss, is designed to serve the needs of residents at risk for exiting the facility or with dementia. Heartwood's physical design and the emotional support provided by staff create an environment of comfort, nurture and security. Staff support resident dignity by working with each individual at his/her own cognitive level. |
